Human Touch Palliative & Hospice Care

Palliative and Hospice Care involves a team approach to give expert medical care for pain management, emotional and spiritual support tailored to meeting the patient's needs and wishes. Human Touch views that patient and family as one unit of care. Both physical and emotional care is extended to both the patient and family members. Comfort and compassion are key to hospice services provided by Human Touch Palliative and Hospice Care.

In order to be eligible for hospice care, the patient's physician in conjunction with Human Touch's medical director will need to certify that the patient has a life limiting disease process, and meets the hospice care criteria. Above all the patient must choose to receive hospice care and not seek aggressive or curative treatment.

A hospice is not a setting but a model of care for those who have a life limiting disease. At Human Touch Palliative and Hospice Care, we are able to provide care in any setting that the patient calls home. Care can be provided in a private residence, skilled nursing facility, assisted living facility, and a board and care setting.
