A death certificate is an official record filed with the Florida State Office of Vital Statistics registering the death of an individual within the state of Florida. It provides proof of death for life insurance claims, Social Security survivor's benefits, veteran's benefits and other transactions needing proof of death.

Long form (with cause) is needed for: life insurance policies, pensions, and Veterans Administration benefits. Short form (without cause) is needed for: stocks/bonds/annuities/trusts, real property, vehicle/boat/airplane titles, Social Security, bank accounts, and credit life insurance.
